- Evidence of malignancy
- Nuclear atypia
- Increased mitotic activity
- Anaplastic histology
- Infiltrative capacity
- Usually treated with aggressive adjuvant therapy.
- Evidence of malignancy
- WHO grade IV
- Mitotically active
- Necrosis-prone
- Proliferation
- Vascular invasion
- Rapid preoperative and postoperative progression and fatal outcomes
